According to the Japan P&I Club, most incidents still stem from navigational errors, often driven by a critical gap:

With waterway management still in transition, prevention relies on disciplined fundamentals:
- structured Master–Pilot passage plan discussions
- verification of updated channels and routes
- heightened awareness of new river stretches with harder bottoms (e.g. Fighiera)
